Carlos Juan Finlay (1833-1915) and the mosquito theory of yellow fever

On 14 August 1881 a physician in Spanish Havana read a paper to the Academy of Sciences proposing that yellow fever passed from person to person inside a mosquito. The room said nothing — no objection, no agreement — and nineteen years passed before an American army board in occupied Cuba acted on the idea and took the credit. Finlay was right about the vector and incomplete about the mechanism: it needed a second discovery, the extrinsic incubation period, that was not his.

Why this wins its question: English-language accounts of yellow fever still tend to run Walter Reed first and Finlay as a footnote; Cuban and Spanish-language accounts often invert it into a story of theft. Neither survives contact with the specialist literature, and this object is built from the part of that literature that is least flattering to the case it supports: a 2024 review by an arbovirologist that lists the men who proposed insect transmission seventy years before Finlay, reports the archival argument that Finlay had read Manson, and explains in mechanistic terms why Finlay's own experiments could not have worked — he was testing mechanical transmission without the extrinsic incubation period that Carter would find in 1898. The 1881 paper, the silence that met it, the nineteen years, the four Nobel nominations and the Wood sentence about justifying the war against Spain are all here too. The claim that survives all of it is narrower than the patriotic version and much stronger than the Anglophone one.

  1. Carlos Juan Finlay Barrés was born at Camagüey, in Spanish Cuba, on 3 December 1833 and died at Havana on 20 August 1915. He graduated in medicine from Jefferson Medical College of Philadelphia in 1855 and validated the degree in Cuba in 1857 at the Royal and Literary University of Havana; his working life was spent as a practising physician on the island, across ophthalmology, tetanus of the newborn, tuberculosis, typhoid, leprosy, malaria and yellow fever.

  2. The theory was announced twice in 1881, in two different registers. On 18 February, as the delegate of Cuba and Puerto Rico to the International Sanitary Conference at Washington, Finlay proposed that yellow fever passed by way of an intermediary agent; on 14 August, at the Academy of Sciences of Havana, he read the paper that named the agent — published that year by the Real Academia de Ciencias Médicas y Naturales de la Habana as "El mosquito hipotéticamente considerado como agente de transmisión de la fiebre amarilla".

  3. The reception was neither refutation nor assent but silence. When the chair invited discussion, the only voice heard was the general secretary's, asking that the work "remain on the table" — the term indicating there would be no comment. None of the academicians present contested the theory and none endorsed it. The account is a Cuban commemorative essay written for the centenary of Finlay's death and reads as such; the particular is dated and specific enough to carry, the surrounding pathos is not treated as evidence here.

  4. Finlay was not the first to suspect insects. John Crawford, an Irish businessman turned physician practising in Baltimore, proposed the involvement of insects in yellow fever from work done in Dutch Guiana in 1807 — some seventy years before Finlay — was ridiculed for it, lost his patients over it, and remains the least recognised of the early proponents. Beauperthuy and Nott made related proposals between the 1850s and 1870. A claim that Finlay invented the idea of insect transmission is not supportable.

  5. Whether Finlay arrived at the mosquito independently is contested, and the contest is worth stating in the terms the specialists use. The French historian and philosopher François Delaporte, after archival work in Cuba, the United States and Europe and interviews with people who knew Finlay's work, concluded that it was impossible for Finlay to have conceived mosquito transmission without having read Patrick Manson. The Cuban-born Aristides Agramonte — later a member of the Reed board and Finlay's friend — placed Finlay's 1881 paper in sequence after Manson's 1870s filaria demonstration and Beauperthuy's articles, which Kuno reads as an ambiguous hint that Finlay knew of both. This is an attribution argument about influence, not about fraud, and it is not settled.

  6. The reason Finlay could not close his own case is technical and is the single most important thing to know about him. Yellow fever cannot be passed by a mosquito immediately: the agent must first develop inside the insect for roughly a fortnight. That interval — the extrinsic incubation period — was identified in 1898 by Henry R. Carter of the US Public Health Service, working along the Mississippi, who noticed an approximately fourteen-day gap between clusters of cases. Before 1900, Kuno notes, what Finlay was actually testing with his mosquitoes was mechanical transmission rather than biological transmission; his inoculations therefore gave results too irregular to compel assent.

  7. What the Reed Commission supplied in 1900 was the combination, not the vector. Kuno's summary is that Finlay's 1881 theory "was pivotal in the historic success of the Reed Commission in 1900", and that the adoption of Carter's extrinsic incubation period "was indispensable" to that success. Neither part of the credit stands without the other, which is why the popular framing — either Reed discovered it or Reed stole it — is wrong in both directions.

  8. Members of the occupying American administration acknowledged the debt in writing, and one of them attached a political conclusion to it that is worth quoting rather than paraphrasing. Leonard Wood, military governor of Cuba, wrote in his government report that "the confirmation of the doctrine of Dr. Finlay is the biggest step that has been given in medical science after the discovery of Jenner's vaccine, and this single fact is enough to justify the war against Spain". William Gorgas, who ran the Havana sanitation campaign and then the Panama one, wrote to Finlay in 1910 that had they followed his instructions on arriving in Cuba they would have obtained in 1899 the results reached in 1901, and that without Finlay's work and his defence of the mosquito theory the American commission would never have undertaken the investigation at all.

Between the paper and the vindication lay nineteen years in which Finlay was treated as a crank — his conclusions, in the words of the centenary essay, taken for "the result of a scientifically feverish and sick mind". His support in that period came from two people: his wife Adela Shine and the Spanish physician Claudio Delgado, his only assistant in the worst years, who had also secured his appointment as delegate to the Washington conference where the theory was first announced.

  10. Finlay was nominated for the Nobel Prize in Physiology or Medicine repeatedly and never received it — in 1904 by Ronald Ross, in 1905 by John W. Ross, in 1907 by Carl Sundberg of the prize committee, in 1912 by the Academy of Sciences of Havana and by a Rio de Janeiro professor, and by Alphonse Laveran jointly with Agramonte in 1912, repeated for 1913, 1914 and 1915. He held the directorship of the institute in Havana that bears his name until his death in 1915, when the German naval physician Wilhelm H. Hoffmann succeeded him.

  11. Finlay was also wrong about things, and the corrections are on the record in the same literature that credits him. He held that Aedes aegypti did not occur at high altitude; the mosquito was in fact present in Mexico City, about 1,700 metres above sea level, in his own lifetime, and has since been found indoors at 2,300 metres. An accurate account of him is of a clinician who identified the right insect for the wrong mechanism and defended it for two decades on evidence that did not yet suffice.

  12. The reach of the result is the practical argument for remembering him. Gorgas carried the mosquito control programme from Havana to Panama, where yellow fever had been among the obstacles to completing the canal; the American Pan-American Medical Congress at Dallas in 1933 adopted Finlay's birthday, 3 December, as the Day of American Medicine. Juan Guiteras Gener, the Cuban public-health figure who mediated the priority dispute between Finlay and the US Army board, said of it that "in that occasion there was glory for all".
